As a support coordination, your primary objective is to empower individuals to achieve their goals by providing them with the necessary tools and guidance. You will work closely with NDIS participants, helping them understand and utilise their NDIS plan effectively. Your responsibilities include:
Understanding and Using the NDIS Plan: It is crucial for you to comprehend the participant’s NDIS plan and help them understand its components, and how to navigate it.
Connecting with Providers and Services: You will assist participants in connecting with NDIS providers, as well as community, mainstream, and other government services that can support their goals and needs.
Building Confidence and Skills: Your role involves building the participant’s confidence and skills in using and coordinating their supports. You will help them develop the necessary abilities to manage service delivery tasks, maintain relationships, live independently, and actively participate in their community.

What we do:
